4Check Hockey, LLC ("4Check," "we," "us") provides hockey contact, checking, and concussion education training to organizations, coaches, players, and families. This policy explains what information we collect, how we use it, and the choices you have.

- Fill out a form on our website, such as a contact form, camp request, organization referral, or session feedback survey

- Sign a service agreement or pay an invoice

Players and minors
Many of the athletes we work with are under 18. We do not knowingly collect personal information from children under 13 through our website. We ask that players under 18 complete surveys with a parent or guardian, and we only share a minor's comments publicly with a parent or guardian's permission. If you believe a child has given us personal information without consent, contact us and we will delete it.
How long we keep information
We keep information only as long as we need it to provide our services, keep accurate business records, and meet legal requirements.
